抄録

A patient with multiple aneurysms of posterior circulation associated with bilateral carotid occlusion is presented.<BR>A 59-year-old man was hospitalized for sudden onset of loss of consciousness.<BR>The patient was semicomatose but marked motor paresis was not present. CT scan showed a subarachnoid hemorrhage and the obstructive hydrocephalus. Cerebral angiogram demonstrated bilateral internal carotid occlusion and multiple saccular aneurysms at the top of the basilar artery and right posterior cerebellar artery (P3). After external ventricular drainage, consciousness gradually improved to become stupor. However, he died in 14th day due to rerupture of aneurysm.<BR>He had history of transient right hemiparesis 10 years ago and left carotid occlusion was suspected. The time of right carotid occlusion was unknown.<BR>In aneurysmal formation, we know hemodynamic stress plays an important role from many literatures. In this case, increase of posterior circulation through bilateral internal carotid occlusion caused hemodynamic stress on basilar and posterior cerebellar arteries and became a factor of aneurysmal formation.
