Effectiveness of the Amsler chart for diagnosing visual impairment in hemodialysis patients: A nursing research study

<p>【Background】Visual impairment reduces the ability of hemodialysis patients to perform activities of daily living. This study aimed to investigate visual impairment and the effectiveness of the Amsler chart for diagnosing visual impairment in hemodialysis patients. 【Methods】 We examined visual impairment with the Amsler chart and investigated patient background factors, symptoms of eye abnormalities, and whether the patients were prepared to undergo eye examinations in 199 hemodialysis patients (mean age, 67.9 years; 125 males and 74 females; mean hemodialysis treatment period, 8.2 years). Visual examinations using the Amsler chart were performed on one eye for each patient. The patients were instructed to stare at the center of the Amsler chart while wearing glasses and standing about 30 cm away from the chart and were evaluated for signs of distortion and blur. In addition, ophthalmologists performed examinations to detect ocular lesions in 40 hemodialysis patients. 【Results】 Sixty-three patients exhibited abnormal Amsler chart results. The number of patients who had diabetes mellitus, high glycated albumin levels, or symptoms of eye abnormalities or suffered from visual distortion of letters was higher among the patients with abnormal Amsler chart results than among those with normal Amsler chart results. Of the 63 patients with abnormal Amsler chart results, 29 patients were willing to consult ophthalmologists, and 34 patients were not. Of the 40 patients who underwent examinations for ocular lesions, 18 had diabetic retinopathy, 8 had macular degeneration, 7 had retinal vein occlusion, and 5 had glaucoma. 【Conclusions】 Approximately 30% of the examined dialysis patients had visual impairment according to the Amsler chart, and most of them had macular disease. The Amsler chart might be a useful test for evaluating visual impairment and help to ensure that hemodialysis patients receive specialized medical treatment for macular disease.</p>
