Infection with human polyomaviruses JC (JCV) and BK (BKV) was investigated in 46 patients with possibly impaired immunity. Urinary specimens containing inclusion-bearing cells suggestive of these viral infections were screened using Papanicolaou stained smears.<BR>Nineteen specimens were selected and processed for in situ hybridization (ISH) using a biotinylated probe for human polyomavirus DNA.<BR>Urinary polyomavirus excretion was detected in 14 of 46 immunosuppressed patients (30.4%). Both JCV and BKV viral DNA excretion was identified in 11 of these 14 patients (78.6%). JCV or BKV excretion alone was found in 2 (14.3%) and 1 (7.1%) respectively. The duration of polyomavirus shedding was approximately four months and resolved spontaneously. By immunocytochemistry, excretion of CMV, ADV, HPV, and/or HPV positive cells was detected in the urinary specimens of patients shedding polyomavirus.<BR>This study confirmed a high incidence of both JCV and BKV excretion in immunosuppressed patients and that the ISH method is preferable for detecting urinary polyoma virus excretion.
