Abstract

Background: We report a rare case of neuroendocrine carcinoma of the common bile duct diagnosed preoperatively by cytology.<BR>Case: A 70-year-old woman with jaundice, upper abdominal pain, and loss of appetite was found in cytology of bile duct brushing and bile to have small round cells in isolation or in clusters in the necrotic background. These cells have a high N/C ratio and scant cytoplasm. The chromatin pattern was granular with ill-defined nuclei. Thin cytoplasm was immunohistochemically postive for synaptophysin, CD56, chromogranin A, NSE, and EMA.<BR>She died of carcinoma with multiple liver metastases 14 months after pancreaticoduodenectomy.<BR>Conclusion: Preoperative cytology of bile duct brushing and bile is valuable in diagnosing neuroendocrine carcinoma of the common bile duct.
