Cytology of follicular thyroid tumors, quality assurance, and differential diagnosis

Objective : We clarified diagnostic accuracy and cytological findings in fine-needle aspiration of follicular thyroid tumors.<br>Study Design : We studied 529 thyroid nodules suspected of having follicular tumors in aspiration cytology. We also reviewed cytological preparation of 122 nodules examined histologically.<br>Results : Nodules reported as benign accounted for 15.2%, indeterminate for 51.0%, and suspected malignancy/malignant for 75.0%. Indeterminate was divided into favoring benign at 40.4 %, intermediate at 58.0%, and favoring malignant at 89.5%. The incidence of follicular carcinoma in cases reported as benign was 10.2% and intermediate 13.6%. Retrospective observation showed that three-dimensional microfollicles and thick trabecular arrangement tended to be observed in follicular carcinoma. No papillary configuration was seen in follicular adenoma or carcinoma.<br>Conclusion : Follicular tumors should generally be reported as indeterminate, but including suspected diagnoses and using subclassifications, such as favoring benign and favoring malignant are helpful in determining surgical indications.
