In the authors' institutes, maxillary skeletal reconstruction following extensive resection of tumors has been carried out according to the principles of restoration of three maxillary buttresses: the nasomaxillary, zygomaticomaxillary, and pterygomaxillary buttresses (NMB, ZMB, and PMB). Their technique, which is the rectus abdominis myocutaneous free flap combined with vascularized eighth and ninth costal cartilages for reconstruction of ZMB and soft tissue defects, was described. The costal cartilages are supplied by the perichondrial vascular network through the anterior intercostal vessels connecting with the deep inferior epigastric vascular system. Restoration of ZMB prevents the inferior deviation of the orbit and provides a good zygomatic contour. This useful technique is considered to reduce the incidence of postoperative complications in maxillary composite reconstruction.
