Abstract

There are many differences between the Japanese Evidenced-Based Guidelines for acute cholangitis and cholecystitis and the International clinical guidelines for acute cholangitis and cholecystitis (‘Tokyo Guidelines’). The present study examines all the differences between them. A remarkable difference in acute cholangitis diagnostic criteria is that the International edition places emphasis on the patient's past history and physical examinations. On the severity index of acute cholangitis, general conditions in the International guidelines are focused on the severity. On the other hand, the category of a suspected diagnosis based on acute cholecystitis diagnostic criteria is lacking. The Japanese edition of the severity index of acute cholecystitis emphasizes the local condition of the right upper quadrant around the gall bladder when deciding whether an emergency operation is warranted. On the contrary, the International edition focuses on the general conditions. As seen above, there are many differences between the Japanese guidelines and the International Guidelines. We should try to compare these characteristic variations between the two sets of guidelines in order to improve the quality of both.
