Are Dilated Submucosal Vessels Viewed under Fiberoptic Bronchoscopy Bronchial Arteries or Bronchial Veins?

In vascular engorgement, one of the common bronchoscopic findings, is merely supposed that the vessels of the bronchial wall are dilated. According to studies of bronchial circulation, the bronchial wall contains not only bronchial arteries but also bronchial veins. This fact is not generally known. In some cases treated at the authors' institutions dilated submucosal vessels of bronchi were suspected to be bronchial veins, for example, cases of mitral stenosis, superior vena caval obstruction etc. If such engorged vessels were bronchial arteries, this finding might be seen in cases with marked development of bronchial arteries. Therefore we studied such cases using fiberoptic bronchoscopy but no case showed vascular engorgement. It was concluded that there is no relationship between vascular engorgement and the development of bronchial arteries. We intend to extend our study to the examination of bronchial vein in cases of vascular engorgement.
