Description
Background. We report a case that required a special approach to close the bronchial stump due to right main bronchial stenosis immediately following VATS right upper lobectomy. Case. A 52-year-old woman given a diagnosis of adenocarcinoma underwent VATS (video assisted thoracoscopic surgery) right upper lobectomy with an ND2a procedure. The bronchial stump was closed with the Sweet technique using an endostapler. Bronchofiberscopic examination before extubation revealed an almost complete obstruction of the right main bronchus. We performed reoperation, removing the endostapler and opening the bronchial stump directly beneath the staple line and closed it by means of the modified Overholt technique. However, we again found obstruction of the right main bronchus. Rereoperation was performed via the right axillar route. After we closed the bronchial stump by means of the modified Sweet techinique using a notch of the posterior membranous wall, the right main bronchus was found to be open. The patient showed a good postoperative course. Conclusion. We need to observe the main bronchus not only from the operation field but also from within the bronchus after closing the bronchial stump.
