In anticipation of the extreme aging of society and as part of its social security measures, the Japanese government has centralized acute phase medical services by differentiating the functions of medical institutions. As a result, acute care hospitals with limited staff need to increase the efficiency of their operating room management in order to provide surgery to an growing number of patients. Various approaches taken to ensure efficient operating room management include changing in-house practices, improving procedures based on scientific evidence, eliminating wasted time, and simplifying tasks. However, safety should not be compromised in the process of increasing efficiency. Anesthesiologists specialized in controlling the fundamental functions of the body should be key persons in the effort to increase operating room efficiency while ensuring patient safety. As a specific measure, we discuss the one-footwear system in the operating room, along with its convenience, spatial and economical advantages, effect on air cleanliness, and safety in terms of surgical site infections. The usefulness of efficient operating room management and the efforts made at our institution are also described. The active participation of anesthesiologists in operating room management is expected to ensure safer perioperative procedures.
