A Report of Three Cases of Right Unilateral Pulmonary Edema after Minimally Invasive Cardiac Surgery for Mitral Regurgitation

Abstract

We experienced three cases of right unilateral pulmonary edema after mitral valve plasty (MVP) course of mitral regurgitation using a minimally invasive cardiac surgery (MICS) technique. A fifty-eight year old male, 51 year old male, and 56 year old female who had no symptoms or findings of heart failure prior to surgery underwent MICS under complete right lung deflation. After weaning off cardiopulmonary bypass (CPB), their oxygenation deteriorated immediately and an amount of viscous and foamy liquid was suctioned from the tracheal tube. The liquid was attributed to the leakage of plasma protein. Oxygenation was improved by continuous high pressure mechanical ventilation and daily administration of ulinastatin and hydrocortisone.<br>We consider right unilateral pulmonary edema after MICS to be caused by the reexpansion of the right lung and ventilation after weaning off CPB.
