The shoulder is a common location for malignant soft tissue tumors. And the deltoid muscles are very important in shoulder function. We performed wide resection of malignant tumors with excision of the deltoid muscle. We report three cases of malignant soft tissue tumors around the deltoid muscle.<BR>There were two women and one man, with a mean age of 73 years. The mean follow-up period was 17 months. Evaluation items were site of origin, pre and postoperative shoulder function (ROM: elevation, abduction, JOA score) and recurrence. There were two tumors within deltoid muscle (extra skeletal myxoid chondrosarcoma 1, solitary fibrous tumor 1) and one tumor existed between the deltoid muscle and the pectoralis major muscle (sclerosing epithelioid fibrosarcoma).<BR>Compared with preoperative range of motion, postoperative range of motion was decreased in all directions (elevation, abduction, external rotation). There was no recurrence. Two cases in which the tumor occured within deltoid muscle kept good shoulder function. On the other hand, the one case of intermuscular tumor resulted in poor shoulder function because of wide resection including partial resection of the rotator cuff.<BR>Reconstruction of the soft tissue around deltoid muscle was very important for the postoperative shoulder function in the case of intermuscular malignant soft tissue tumor which occurred near deltoid muscle.
