Description

  Bupivacaine has a pipecoloxylidide skeleton and exists as S (-) and R (+) enantiomers (optical isomers) levobupivacaine and dextrobupivacaine, respectively. Marcain® (bupivacaine) is racemic with an enantiomeric ratio of 50: 50 ; however, the novel anesthetic Popscaine® is pharmaceutically formulated from only the S (-) -enantiomer, levobupivacaine. While both enantiomers exhibit concurrent activity, levobupivacaine has greater activity than dextrobupivacaine and causes less cardiovascular compromise. Of the pharmaceutical products approved in April 2008 in Japan, 0.25% product acquired effect-efficacy for postoperative analgesia (epidural administration) and 0.75% one acquired effect-efficacy for epidural anesthetic (peroperative period) .
