A 74-year-old woman was found to have low serum urate concentration(1.93±0.36mg/dl)associated with diabetes. Her urate clearance was elevated(17.7±7.1ml/min)and did not change after administration of pyrazinamide or benzbromarone, but was increased after administration of probenecid. Hypoxanthine clearance was not suppressed by treatment with pyrazinamide, and was increased by probenecid administration. However, hypoxanthine clearance was decreased by treatment with benzbromarone. Xanthine clearance was increased by treatment with probenecid but not affected by treatment with benzbromarone.<BR>We suggest that this patient had a renal abnormality, which may have been due to diabetes,which affected tubular presecretory reabsorption of urate and oxypurine.
