A clinicopathologic study was performed in 53 cases of hepatocellular carcinoma (HCC) with sarcomatous appearance seen among 550 autopsy cases of HCC. The incidence of HCC with sarcomatous appearance is increasing, and anti-cancer therapies, such as one-shot injection therapy and transcatheter arterial embolization, seem to be one of causes of sarcomatous change. Clinically, the cases with sarcomatous appearance are not significantly different from those without sarcomatous change. Pathologically, however, most of the cases with sarcomatous appearance are grossly accompanied by varying degrees of infiltrative growth, and pedunculated growth is frequent. Histologically, the part of sarcomatous appearance represents sinusoidal growth pattern at the tumor-nontumor boundary.<BR>Both hematogenous and lymphatic metastases are more frequent in the cases with sarcomatous appearance than usual cases. As to the mechanism of the occurence of sarcomatous change in HCC, it is predicted that the change of the phenotype of the cancer or the selection of the clone characterized by sarcomatous nature may result from the active anti-cancer therapy.
