An autopsy case of veno-occlusive disease of the liver was reported. The patient, 39-year-old female, received mitomycin C therapy (72 mg in total) after resection of ileo-cecal tumor. Two months after beginning of mitomycin C medication, edema, intractable ascites and hypoproteinemia developed. She died of the rupture of esophageal varices four months later. Postmortem examination revealed prominent congestion of the liver (nutmeg liver) without forming thrombus in the hepatic vein. Microscopically, the many of the central veins and of the sublobular veins were stenosed or occluded by the marked fibrous thickening of the intima. These were quite similar with the veno-occlusive disease in West India. In the present case, mitomycin C was regarded as the only possible causative agent of the veno-occlusive disease, because there was no history of radiation and organ transplantation. Mitomycin C is widely used as an anti-cancerous drug, but the present case suggests much attention should be needed when mitomycin is prescribed.
