Abstract

We report a case of hepatitis C related cirrhosis with genotype 1b with NS5A resistance-associated variants (RAVs). He was treated with the all-oral combination of the nucleotide polymerase inhibitor sofosbuvir (SOF) and the NS5A inhibitor ledipasvir (LDV) for 12 weeks. Although the HCV-RNA in his blood showed negative conversion four weeks after the start of treatment, it relapsed after the treatment for four weeks.

Previous reports that patients with baseline NS5A RAVs at baseline which treated with SOF/LDV had rates of SVR12 that were numerically and rates of virologic failure that were numerically higher than patients without NS5A RAVs.

We should pay attention to the NS5A RAVs at baseline in case of treatment for hepatitis C with SOF/LDV.
