抄録

Forty American cases of noncirrhotic acute alcoholic hepatitis were studied. Black female patients predominated in number. Female patients were significantly younger than male patients. The characteristic clinical features included abdominal pain, fever, icterus, very large liver, splenomegaly and ascites. SGOT elevation, generally of moderate degree, was present in the all patients. SGPT elevation was not so frequent, and was mild, while in the comparison the elevations of SLDH and total serum bilirubin were higher in both incidence and degree. The centrilobular area was the site of the major pathology characterized by alcoholic hyalins, swelling and lytic necrosis of liver cells, neutrophilic infiltration, fatty metamorphosis, and in a half cases, centrilobular fibrosis. The centrilobular fibrosis was an important finding, because it accompanied severer liver-cell injury and portal hypertension, and appeared an obligatory step in the evolution into monolobular regular cirrhosis. Occasionally they simmulated clinical pictures of obstructive jaundice and acute viral hepatitis.
