The patient was a 56-year old man who developed diabetes at age 20. The disease was ill-controlled. From 10 days before he was first seen, a severely painful reddish swelling was rapidly spread over the dorsum foot due to the secondary infection of interdigital tinea of the right foot. The lesion was not responded to a high dose antibiotic therapy, and 20 days after the onset of the lesion an extensive debridement was performed. The total layer of subcutis was found necrotized with neutrophilic infiltration. The extensor tendon was necrotized, and a part of muscle was also necrotic. The resected site was kept opened under a wet to dry dressing until the appearance of granulation; 26 days after operation, the right fourth toe was transected, and the Thiersch grafting was instituted. Subsequently, after the blood flow was improved by a patch extension, the skin grafting was repeated; 180 days after hospitalization, the whole part of the involved skin was successfully grafted and patient was discharged.
