A case of atopic dermatitis suspected to be induced by egg allergy.

We experienced a case of a child who suffered from atopic dermatitis strongly suspected to be induced by egg allergy. The patient was a 18-months-old female child who developed a so-called seborrheic dermatitis-like eruption on her head one month after birth and the eruption extended to the trunk thereafter. She first visited our clinic at one year and 6 month old, and treated by steroid ointment therapy. From her present illness her symptoms and signs considered to be worsend by food intake which contained egg. Therefore provocation test with egg was performed on admission. Her RAST score of egg white was class 3. When she was first challenged with one tenths of a whole egg, eruption and itching appeared 6 hours later. The same symptoms and signs re-appeared 4 hours after the second challenge with one thirds of a whole egg. And on the third challenge of awhole egg, she felt severe itching 10 minutes later, and erythemas and papules developed on her trunk and limbs one hour later. Her eruption reached to the peak after 6 later, faded gradually the next day and only eczematous lesion remained. Patch test with ovalubumin was positive. This case of atopic dermatitis is strongly susupected tobe induced by egg allergy, which may be neither classical immediate nor delayed type of allergy but an atypical allergy with delayed time course such as a Jones-Mote reaction or a late phase reaction.
