Abstract

A 57-year-old male with congenital phimosis noticed cauliflower-like tumor in the tip of penis, and the tumor increased to the size of an infant' s fist after 6 months and covered the urinary meatus. Biopsy of the tumor showed SCC and the palpable inguinal lymph nodes showed no metastasis. Because marked anteflexion was observed, phallectomy was not performed, in consideration of the QOL. Instead, penile ischemic therapy reported by Murahashi et al. was performed in combination with ODT of such as Bleo ointment. The tumor showed necrosis and exfoliation after 3 weeks, and the normal glans was exposed. Because the tumor originated in the internal lamina of the prepuce and it covered the glans, the detemination of the initiation site of the tumor was difficult at the initial consultation. No recurrence has been observed for 2 year and 2 months after the ischemic therapy. We considered the ischemic therapy was useful for this case, because he was free from phallectomy.
