A 28-year-old woman developed warty papules on the palms and soles 4 months earlier. These were left untreated and seemed to subside temporarily. However, multiple, round and flatly protruding, lustrous and red-brownish papules began to appear on the backs of both hands, extensor-sides of the forearms and dorsal aspects of both feet. The superficial lymph nodes were not swollen. A diagnosis of papular secondary syphilis was established from the clinical and serological findings and the response to treatment with benzylpenicillin benzathine. Histological examination revealed the presence of only a very small number of plasma cells but a necrobiotic structure resembling palisading granuloma in the middle layer of the dermis. Secondary syphilis with such a histological structure has so far been reported only once. Because the present case was considered very rare, it was presented together with a review of the literature.
