A case of epithelioma cuniculatum in a 58-year-old male is reported. He noticed a tender tumor developed at the site of clavus on the right sole one year ago. When he first visited our clinic, he could not walk due to pain by infectious change of the lesion. A keratotic papillomatous tumor of about 5 cm in diameter with ulcer was noted on the posterior plantar surface of the right sole. The surroundings of the tumor showed red swelling. Purulent and sebum-like materials with offensive odor were excreted from the bottom of this tumor.<BR>Histopathologically it showed traits of a squamous cell carcinoma of low grade malignancy and an extreme keratinization beside the typical numerous anastomosing crypts similar to rabbit warrens.<BR>The excision of this tumor and the transplantation of the free autograft were performed.<BR>HPV type 16 DNA was detected in archival formalin-fixed, paraffin-embedded tissue sections using polymerase chain reaction with consensus primers, but HPV type 16 DNA was negative by in situ hybridization. Immunohistochemically HPV capsid antigens were also negative.
