Description

An eight-month-old girl was admitted with bronchiolitis and severe respiratory distress due to RS virus. Chest X-ray showed severe right upper lobar emphysema and atelectasis of the right lower lobe. Computed tomography revealed a right-sided tracheal bronchus. The patient was referred to us and evaluated by bronchoscope; an accessory bronchus communicating to the right lateral wall of the trachea 11mm proximal to the carina was identified. A check valve mechanism due to malacia was demonstrated. At 13 months of age she underwent right upper lobectomy without incident. Sections of the tracheal bronchus revealed normal cartilage and a bronchial lumen. She subsequently had an uneventful postoperative course. For the diagnosis, the presence of a tracheal bronchus should be considered in children with recurrent pneumonia and lobar emphysema. We believe that a multidetector computed tomography (MDCT) scan of the chest is useful.
