Abstract

A two-year-and-seven-month boy was admitted to our hospital because of sudden abdominal pain and vomiting. Abdominal X-ray revealed large amounts of free air. An emergency operation was performed under a diagnosis of septic shock due to gastrointestinal perforation. Intra operative findings confirmed a 7-cm-long rupture along the greater curvature of the gastric cardia with ischemic changes of the abdominal esophagus. The spleen was not fixed. These findings indicated acute gastric volvulus with gastric rupture due to a wandering spleen. In addition to closure of the gastric rupture, gastrostomy and gastropexy were performed to prevent recurrent torsion. Gastrostomy is a useful method in fixing the stomach.
