Description

A 34-year-old woman was admitted to our hospital because of severe abdominal pain which developed while playing beach volleyball. Abdominal US and CT scan revealed a huge cystic tumor 16 cm in diameter in the upper abdomen and massive ascites. Emergency laparotomy was performed under a diagnosis of panperitonitis due to the rupture of the cystic tumor. Laparotomy showed massive serous ascites and the ruptured cystic tumor which compressed the surrounding organs. Direct closure of the ruptured tumor and abdominal drainage were performed. On the 17th postoperative day, distal pancreatectomy including the tumor was performed under a diagnosis of mucinous cystic neoplasm of the pancreas. The pathological diagnosis was mucinous cystadenoma of the pancreas with ovarian-type stroma. Ruptured mucinous cystic neoplasm is a comparatively rare entity. This case of the disease causing peritonitis is so rare that only other two cases have been reported in Japan.
