Description

A 63-year-old woman had periodically been followed for a cyst 21mm in diameter with a septum in the lateral segment of liver which had been detected by abdominal ultrasonography 17 months earlier. Abdominal ultrasonography revealed an increase in the cyst size to 43×25mm in diameter as well as the appearance of a solid lesion in the cyst. Based on abdominal ultrasonography, abdominal CT scan, magnetic resonance imaging scan, and angiography, hepatobiliary cystadenocarcinoma was diagnosed. Operative procedures included a lateral segmentectomy and cholecystectomy. The resected material showed that the tumor was a well-defined, multilocular and cystic lesion, its inner part was filled with multilocular brown mucus, and in one part, a white papillary lesion grewing towards the lumen. Histopathologically the cyst wall was hepatobiliary cystadenoma composed of columnar epithelium, and the papillary lesion in the lumen was diagnosed as hepatobiliary cystadenocarcinoma because structural and nuclear atypia was identified.<br> This paper present a clinicopathological discussion of this case of hepatobiliary cystadenocarcinoma which was diagnosed by imaging methods during clinical observation of a hepatic cyst and was completely removed, together with some bibliographical comments.
