Abstract

An 87-year-old woman was admitted in our hospital. A colonoscopic examination suggested the presence of a 5cm whole circumferential tumor in the rectum 7cm above from anal verge. Biopsy diagnosis was group V, and a diagnosis of the rectal cancer was made. Low anterior resection was carried out. During the operation a tumor was found in the ileum near the ileocecal junction, when and therefore, the preoperative barium enema was reviewed again. A 0-IIa like lesion was confirmed in the barium enema. Since the possibility of malignancy can not be denied, the ileocecal resection was added. Pathohistologically, the tumor in the rectum was diagnosed as moderately differenciated Type 2 ss INF, β ly1 v0 n0, adenocarcinoma and tumor in the ileum was diagnosed as well differenciated Type 0-Is sm2 INF, β ly0 v0 n0, adenocarcinoma. Synchronous double cancer of jejunoileum and large intestine was rare and 12 cases are reported in this country up to now. In this case, it was fortunately diagnosed in the early stage, and the curative operation was performed. Preoperative careful evaluation of small intestine for possible malignancy may be imporlant.
