Description
Chylothorax has remained a difficult clinical challenge for which the ideal treatment has not yet been well established. A 23-year-old female with idiopathic chylothorax who underwent successful surgical treatments is reported. The patient was admitted to the hospital because of massive left pleural effusion. Thoracocentesis revealed chyle amowting to 1, 500ml per day but the etiology was not known by examinations Conservative treatments such as fasting, intravenous hyper-alimentation and a continuous drainage were performed for 10 days without remarkable improvement. She underwent thoracascopy-aided clipping of the thoracic duct. The postoperative course was uneventful, and no recurrent pleural effusion has been recognized for 5 years since then. Because there were no underlying diseases to cause chylothorax, in this patient, the diagnosis of idiopathic chylothorax was made. Clipping of the thoracic duct was proved to be a safe and effective procedure. When conservative treatment of chylotholax is not working, early surgical intervention and clipping of the thoracic ducts are recommended.
