Abstract

A 24-year-old woman driving under the influence of alcohol without a seatbelt collided with a car from the opposite lane. She was found wedged between the steering wheel and front seat. She was rescued and transported to the nearest hospital. Abdominal radiography showed right posterior hip dislocation, right acetabulum fracture, and bilateral pubic bone fractures. Blunt thoracic aortic injury (BTAI) was diagnosed by computed tomography (CT) scan. She was transferred to our hospital for surgical management of BTAI after correction of right posterior hip dislocation, and graft replacement of the thoracic aorta was performed on the same day. A CT scan and ultrasonography could not identify the source of hemorrhage causing hemoperitoneum ; hence, exploratory laparotomy was performed. Laparotomy revealed left ovary rupture. Pathological examination of the ruptured wall indicated an ovarian cyst. Thus, injury to the ovaries needs to be considered in a female patient with multiple injuries and hemoperitoneum of unknown cause.
