A Case of Myxofibrosarcoma Arisen from the Greater Omentum

Abstract

A 61-year-old woman who noticed an abdominal tumor was referred to the department. An abdominal contrast-enhanced computed tomography showed a 10×10 cm cystic tumor without enhancement effect on the dorsal aspect of the stomach examinations included MRI and abdominal angiography offered no definite diagnosis. Therefore, surgery was conducted for the purpose of treatment and diagnosis. The tumor existed in the omental bursa. There was no invasion into other organs. The only connected greater omentum was surgically removed with the tumor. The histopathological diagnosis was primary myxofibrosarcoma (MFS) of the greater omentum. In the 11th month after the surgery, recurrence was confirmed at almost the same place as before the surgery, and we performed second surgery. As the tumor had invaded many organs including the posterior wall of the stomach, pancreatic body and tail, and transverse mesocolon, a tumor resection in bulk was performed through total gastrectomy, extended right hemicolectomy, distal pancreatectomy, splenectomy, and cholecystectomy. However, in the third month after the second surgery, a tumor which was considered to be seeded on the liver surface was confirmed. Despite chemotherapy, she died 25 months after the first operation. This paper deals with this extremely rare case of primary MFS of the greater omentum with some bibliographical consideration.
