Description
A 43-year-old female patient with cholecystolithiasis and a right-sided round ligament underwent single-incision laparoscopic cholecystectomy, utilizing indocyanine-green (ICG) fluorescent cholangiography to identify the anatomy of the extrahepatic bile ducts in real time. As a fluorescent source, 1 ml of ICG (2.5 mg/ml) was injected intravenously after intubation in the operating room. The laparoscope of the fluorescent imaging system was introduced through a subumbilical trocar and fluorescent cholangiography was performed by changing the full color images to fluorescent images using a foot switch. Initially, fluorescent cholangiography failed to differentiate the cystic duct from the common hepatic duct, suggesting retraction of the gallbladder to expose the triangle of Calot to be insufficient. The gallbladder was lifted cranially, allowing clear identification of the cystic duct as well as the common hepatic duct on the fluorescent images.
