A 76-year-old woman presented to the emergency room with sudden abdominal pain. There was no rebound tenderness. The white blood cell count was 5,950/μL. Contrast-enhanced abdominal computed tomography revealed thickening of the small intestine and ascites in the right paracolic space without free air in the abdominal cavity. After a few hours, the patient's abdominal pain worsened and rebound tenderness appeared ; thus, an emergent laparotomy was performed. Intraoperatively, perforation of ileal diverticulum was identified ; ileocecal resection and abdominal lavage were carried out. The patient had an uneventful postoperative course and was discharged on the 15th postoperative day. On pathology, perforation of pseudo-ileal diverticulum and inflammation were noted.<BR>Perforation or penetration of ileal diverticulum is rare. Ileal diverticulosis often occurs on the mesenteric side. However, in our case, the perforation was unusually located on the anterior wall of the ileum.
