Spontaneous rupture of the spleen in a case of primary splenic malignant lymphoma

Abstract

To date, the Index Medicus lists 13 cases in which primary splenic malignant lymphoma caused rupture of the spleen.<BR>The patient was a 76-year-old man in whom left lateral region pain had appeared suddenly.<BR>The accumulation of liquid was observed on the surface of the liver and on the circumference of the spleen on abdomen-imaging CT, and a round tumor with a maximum diameter of 7 cm was observed inside the spleen.<BR>We agreed that the high absorption region where there was external blood vessel leakage suggested periphery of a tumor ; it was judged to be intra-abdominal bleeding from the burst splenic tumor, and urgent IVR was performed. <BR>After TAE, from laboratory findings and an abdomen examination, we determined that the arrest of hemorrhage was insufficient, and conducted an emergency operation the next day. <BR>Tunic damage and bleeding were observed in the upper spleen, and spelenectomy was enforced.<BR>The cuisurface of the tumor was a white solid tumor.<BR>Pathologic diagnosis was non-Hodgkin diffuse large B-cell lymphoma. <BR>Postoperative progress was good, and the patient was transferred to chemotherapy, and is still alive 6 months after the operation.
