Description

A case of rectal metastasis from sigmoid colon cancer is reported here. The patient was a 52-year-old woman. For the advanced sigmoid colon cancer a sigmoidectomy with D3 lymphadenectomy was performed. Pathology of the resected specimen showed stage IIIb (T3N2M0)-moderately differentiated adenocarcinoma. Single liver metastasis detected seven months after primary surgery was treated by partial resection of the liver. One year after the primary surgery, a surveillance colonoscopy revealed an elevated tumor of the rectum. Pathology of the biopsy specimen was moderately differentiated adenocarcinoma, which was similar to the sigmoid colon cancer. At the same time, multiple lung metastasis was detected. For the diagnosis and local treatment, we performed a trans-anal local excision for the rectal tumor. The tumor was located in the anterior wall of the rectum and invaded the submucosal layer and muscular layer. Pathological examination of the resected specimen revealed moderately differentiated adenocarcinoma similar to the sigmoid colon cancer. The carcinoma existed mainly in submucosal and muscular layer with a very small mucosal component. The rectal tumor was therefore thought to be a rare distant rectal metastasis from sigmoid colon carcinoma.<br>
