Reproducible Study for Endoanal Ultrasound for Patients with Anal Sphincter Insufficiency

Endoanal ultrasound is a safe and convenient examination for patients with fecal incontinence. The main aim of this study was to investigate the reproducibility of findings obtained using endoanal ultrasound. After the initial investigation, we evaluated whether these reproducible findings were correlated with manometric findings. Data for 33 women suspected of having anal sphincter insufficiency were re-evaluated by four technicians participating in this study. Reproducible findings were not obtained with regard to the external anal sphincter. On the other hand, all four examiners agreed on the findings for the internal anal sphincter in all the cases. The reproducibility rate of the “interruption” findings for the internal sphincter was 76%, while that of the “uneven thickness” findings was 61%. The manometric pressure data (mean resting pressure (MRP) and mean squeeze pressure (MSP)) were compared between cases with and those without the above-mentioned findings. Cases with and those without “interruption” findings had MRP values of 40.2 cmH2O and 57.2 cmH2O (P=0.04), respectively, while cases with and those without “uneven thickness” findings had MRP values of 43.9 cmH2O and 66.8 cmH2O (P<0.01), respectively. Cases with and those without “interruption” findings had MSP values of 81.3 cmH2O and 118.6 cmH2O, while cases with and those without “uneven thickness” findings had MSP values of 109.6 cmH2O and 135.3 cmH2O, respectively. The internal anal sphincter findings obtained using endoanal ultrasound were highly reproducible and might predict anal manometric function.
