Description

A 75-year-old man who underwent endoscopic subcutaneous dissection (ESD) for early gastric cancer was referred to our hospital for the purpose of undergoing additional resection. Preoperative contrast-enhanced CT scan showed an anomaly of the portal venous system. Besides the main trunk of the portal vein, the left gastric vein had formed another portal vein via the right gastric vein which drained into the caudate lobe, left lobe and posterior region. The hepatic artery demonstrated narrowing caused by arteriosclerosis, so that we decided to preserve the portal vein and performed operation. Laparoscopic total gastrectomy was performed under general anesthesia. We separated off the portal vein from the right to the left gastric vein and the whole length of it was preserved. After the operation, hepatobiliary enzymes increased, but he was discharged from our hospital on the 10th postoperative day.<BR>Anomalies of the portal vein are very rare and we have encountered few cases in the literature. So far such a case as we present here has not been reported, where imaging diagnosis using multi detector-row CT (MDCT) was helpful to grasp the courses of vessels.
