A 16-year-old male was admitted to our hospital with abdominal pain, diarrhea, high fever, and diffuse peritonitis. He had eaten half-cooked meat about seven days previously. Abdominal CT showed peritoneal fluid collection and paralytic Hens.<BR>He was taken to the operating room with a presumptive diagnosis of panperitonitis. At surgery, phlegmonous colitis was found, with perforation of the caecum. A right-side colectomy was performed.<BR>After the surgery, a stool culture, examined preoperatively, was positive for Escherichia coli O-153. Thus, the patient was diagnosed as having diarrhogenic colitis caused by Escherichia coli O-153. Histologically the resected colon demonstrated a pathy mucosal defect, diffuse transmural phlegmone, and numerous venous thrombi in the submucosal layer.<BR>He made a complete recovery, with intensive postoperative care.
