A Case of CK7+/CK20-/CDX2- Transverse Colon Cancer that was Difficult to Differentiate from Metastatic Prostate Cancer

An 83-year-old man who had undergone hormone therapy for prostate cancer with bone metastasis presented to us with a 1-month history of hematuria and hemorrhagic stools. Cystoscopy revealed nodules in the bladder suggestive of bladder cancer. Bladder biopsy revealed findings compatible with a metastasis from the prostate cancer. Then, colonoscopy revealed marked narrowing of the lumen in the middle of the transverse colon. A colonic biopsy from this region revealed poorly differentiated adenocarcinoma. CT showed no metastasis to the lung, liver or abdominal lymph nodes. Therefore, we made the diagnosis of primary transverse colon cancer complicated by prostate cancer invasion of the urinary bladder, and performed partial resection of the colon. Immunohistochemically, the tumor was positive for CK7 and negative for CK20 and CDX-2, a pattern that was not typical of primary colon cancer ; however, additional immunostaining revealed that the findings were compatible with the diagnosis of primary colon cancer.
