Description

In perianal area, except very popular diseases of p'erianal abscess and anal fistula, there occur three kinds of infectious disorder i.e., pilonidal disease, hidradenitis suppurativa and cellulitis. For the past 5 years, we experienced 33 cases of pilonidal disease 6 cases of hidradenitis suppurativa and 5 cases of cellulitis.<BR>The lesion of pilonidal disease has its characteristics of controversial etiology, location (sacrococcygeal), pathology (chronic inflammation with hair) and chronic course. Only by the complete resection of the fistula, the cure is attained. The lesion of hidradenitis suppurativa has its characteristics of location (gluteal area, often bilateral) and pathology (multiple infection of apocrine glands and subcutaneous tissues) and the course (chronic). As the surgical procedures the complete resection of the infected skin are also required. The open wounds made by the surgical resection in those 2 diseases are remained as it is (opened) or primarily closed (sutured) depending on the preference of the surgeons, of which we recommend the latter (closed) procedure. The third and last disease of cellulitis has its characteristic in the origin (perianal abscess), virulence (outen mixed infection of aerobic and anaerobic), progress (rapid), extension (extending to whole body) and severity (systemic symptoms and endotoxin shock). Prompt surgical intervention is the most needed procedure for the rescue of the patient, foll-owed by suitable antibiotics, transfusion and preventive measure for shock.
