Significance of perinearal invasion in colorectal cancer

Vessel infiltration will be one of the factor affected to recurrence of colorectal cancer after surgery. Peri-neural invasion (pni) which is the most important prognostic factor in pancreatic cancer will be also affected to colorectal cancer prognosis.<BR>We analysed 406 cases (184 colon, 222 rectum) of colorectal cancer in our department. Perineural invasion positive rates were pni(+)pm 16.3%, pni(+)ss 12.0% in colon cancer and pni(+)pm 5.6%, pni(+) ss 13.1% in rectal cancer. Cases of pni(+) in colon cancer had high probability of lymph node metastasis and those in rectal cancer had high probability of liver metastasis, lymph node metastasis and local recurrence. It was revealed that perineural invasion, especialy in sub-serosal sheath invasion affected to the prognosis of colorectal cancer.
