From June 1979 to May 1980, fecal occult blood testing with guaiac impregnated filter paper slides (Shionogi slide B) was performed in 1003 subjects under a restricted diet on two succssive days as screening for colorectal cancer at the outpatient clinic. Follow-up over a three-year period after thorough diagnostic investigation was completed in 936 subjects.<BR>The overall positive rate for the test was 20 percent. The sensitivity and specificity for the detection of the source of gastrointestinal bleeding were 71 and 92 percent, respectively.The predictive value was 67 percent for the source of bleeding throughout the gastrointestinal tract, 38 percent for gastrointestinal neoplasia and 18 percent for colorectal neoplasia. The sensitivity for the detection of colorectal cancer was 90 percent in 20 patients. The test was positive on both of two consecutive specimens in 15 of 18 patients with colorectal cancer who had positive results and in six of seven patients with early clorectal cancer who had at least one positive slide.<BR>From this prospective study, it is concluded that fecal occult blood testing with Shionogi slide B on two successive days is very useful as a screening procedure for the early detection of colorectal cancer.
