The incidence of portal vein tumor thrombosis of metastatic liver tumors has been rare. This paper reports a patient who was recieved left hepatic lobectomy for a liver metastasis of the rectal cancer with left portal vein thrombosis and has been survived 2 years and 5 months after hepatectomy without recurrence.<BR>A 54-year-old man was recieved rectal amputation for a rectal cancer on Oct. 4, 1989 and pathological findings of the rectal tumor was revealed moderately differentiated adenocari-noma with serosal invasion, slight lymphatic permiation, marked venous invasion and the grade n2 of lymphnode metastasis resulting in absolute culative resection being made. In Aug. 1990, the serum CEA was increased and a solitary liver metastasis with left portal vein tumor thrombosis was demonstrated by abdominal CT and US. On Oct. 31, 1990, left hepatic lobectomy was made and a liver tumor of 7 cm in diameter with continuous left portal vein thrombosis of 3 cm in diameter was obtained. The tumor was confirmed a liver metastasis of rectal cancer pathologically.<BR>In the past reports, the prognosis of the metastatic liver tumors with portal vein tumor thrombosis were poor, because almost all such cases were far advanced. But this case suggests that a patient whose liver tumor with portal thrombosis being completery resected may be survived for a long time after operation.
