It is sometimes difficult to identify a primary internal opening of anal fistula by inspection and digital examination. Therefore, the injection of povidone iodine & hydrogen peroxide from secondary openings was assessed as a diagnostic option. Eighty-five cases out of 114 (74.6 %) were identified a primary internal opening by inspection and digital examination and 60 cases out of 114 (52.6%) by povidone iodine & hydrogen per-oxide. Twenty-nine cases were not identified by inspection and digital examination, but 12 cases out of 29 (41 %) were identified by povidone iodine & hydrogen peroxide. The rate of identification of primary internal openings of anal fistula increased from 85/114 (74.6%) to 97/114 (80.7%) (P>0.05). Forty-four cases out of 48 (92 %) showed the same location of a primary opening, whereas 4 were not coincident cases which included Crohn' s disease and recurrent fistula by the two methods. Thus, this povidone iodine & hydrogen peroxide method contribute to improvement and confidence in diagnosis of a primary opening of anal fistula.
