One hundred ten patients (7 males, 103 females) of renal failure, due to lupus nephritis undergoesing dialysis treatment in Japan were surveyed. Of 110 patients, 41 cases were still recieving hemodialysis at the survey time of February 1978, 59 cases were already dead and 10 cases have been completely escaped from dialysis returning to the previous conservative treatment. The renal symptoms at the onset of dialysis could divide into 3 types; rapidly progressive renal failure 36, decompensated chronic renal failure 58, and nephrotic syndrome 15. Almost all of these cases showed active immunologic activities, such as positive ANF, positive anti-DNA antibody or depressed serum complement levels. However, the levels of serum Cr, BUN, serum K and hematocrit were not always unanimous in these cases. Steroids were given in 90 cases out of 110 cases at the start of dialysis and this treatment was needed to continue to depress the lupus activity in 86% of all cases. The most frequent cause of death during dialysis treatment was infection, such as sepsis or pnumonia. From this survey it is concluded that 1) the indication for the dialysis should be determined according to the renal symptoms and immunologic activities, 2) Infections should be carefully avoided during dialysis treatment, and 3) appropriate dosis of steroids should be given in some cases during dialysis treatment, particulary when there is possibility to stop the regular dialysis by this pharmaceutical procedures.
