This study was designed to determine the relationship between plasma dopa and total dopamine levels, and clinical signs of patients with Parkinson's disease during levodopa treatment, especially rigidity, bradykinesia and abnormal involuntary movement (AIM). Also combined treatment with dopa decarboxylase inhibitors (MK486 or Ro4-4602) and levodopa was compared with levodopa alone treatment. On administration of the clinical optimal levodopa dose, the plasma dopa levels showed no significant difference between the regimens in patients with Parkinson's disease. But in parkinsonian patients whose onset of the disease until forty years of age, a higher absorption of levodopa in the intestine than in parkinsonian patients whose onset of the disease after forty years of age, was indicated.A close relationship was found between the peak levels of plasma dopa and the appearance of AIM. This finding suggests that both the amount of dopa delivered to the brain and the response of striatal receptor sites to dopamine are important factors in the production of AIM.
