Glucagon is known to enhance insulin secretion directly and has been given to patients for glucagon loading tests (GT) to determine the reserve function of the islet cells. In the present study, we evaluated GT in 8 diabetics given diet therapy (diet group), 11 diabetics on insulin treatment (insulin group) and 16 non-diabetics. The results were compared with those of the 75-g oral glucose tolerance test (75g OGTT) with special reference to the serum C-peptide reactivity (CPR) levels. The data obtained were as follows: 1) Fasting CPR was, significantly lower in the insulin group than in the diet group and in non-diabetics. (p<0.01). 2) The peak level of CPR in GT was 5.9±2.8ng/ml (mean±S. D.) in non-diabetics, 3.6±0.9 ng/ml in the diet group and 1.0±1.0ng/ml in the insulin group (non-diabetics v. s. diet group p<0.01, non-diabetics v. s. insulin group p<0.001, diet group vs. insulin group p<0.001). 3) The peak level of CPR in GT showed significant correlation with the peak level of CPR in 75g OGTT in non-diabetics (r=0.56, p<0.05) and in diabetics (r=0.85, p<0.001). 4) In diabetics, a significant correlation was observed between the peak level of CPR in GT and urine CPR (r=0.80 p<0.001). 5) HbAi and fasting blood plasma glucose were not correlated with the peak level of CPR in GT. These data suggest that GT is a useful test for evaluating the insulin-secreting capacity in diabetics.
