Serum trypsin concentrations in 35 normal controls and 315 diabetics were determined by radioimmunoassay using polyethylene glycol precipitation methods (CIS Sorin) The following results were obtained.<BR>1) As the fasting serum glucose concentrations became higher, so the immunoreactive trypsin (IRT) levels in the serum of diabetics became lower. A negative correlation was thus found between the serum IRT and fasting serum glucose (r=0.25).<BR>2) The serum IRT levels during 75 g glucose tolerance tests in 24 untreated diabetics did not change with increase in the serum glucose after glucose loading.<BR>3) In the three groups of non-insulin dependent diabetics treated with diet alone (n=49), with diet plus oral hypoglycemic agents (n=75), and with diet plus insulin (n=86), the fasting levels of IRT were 26.5± 10.5 (M+SD), 28± 10.6, and 23.0± 9.5 ng/ml, respectively. These levels were significantly lower than that (36.0± 9.1 ng/ml) in 15 normal controls aged over 35 years (P<0.05).<BR>4) The serum IRT of 37 insulin dependent diabetics was 12.5± 8.0 ng/ml and that of 7 diabetics with pancreatic disease was 12.5±10.0 ng/ml. These IRT levels were significantly lower than that (29.7±8.1 ng/ml) of 20 normal controls aged under 35 years (P<0.001).<BR>5) The levels of serum IRT appeared to be elevated in 10 patients with diabetic nephropathy.<BR>6) Diabetic retinopathy and the duration of diabetes did not affect the concentrations of serum IRT in the diabetics tested in this study.<BR>7) Aging and a habit of drinking appeared to increase the serum IRT in both the diabetics and normal controls.
