A 19-year-old woman, who had a history of type 1 diabetes mellitus since 15 years of age, was admitted to our hospital due to diabetic ketoacidosis (DKA) in November 1997. She had hepatomegaly and liver dysfunction on admission, but the abnormalities resolved after administration of insulin. However, marked hepatomegaly and liver dysfunction reappeared with hypoglycemia, hypokalemia, and leucocytopenia 4 weeks after DKA. Liver biopsy showed extensive glycogen deposition and mild steatosis. Her hepatomegaly disappeared in acoordance with a reduced insulin requirement 10 weeks after DKA. However, she showed recurrent hepatomegaly 5 months thereafter, consistent with poor diabetic control. In our case, glycogen storage may have been promoted by high concentrations of glucose, as well as pH change and release of glucose toxicity. Marked hepatomegaly caused by glycogen storage with type 1 diabetes mellitus is unusual. Thus, we speculate that such cases may have gene mutations in enzymes related to liver glycogen metabolism.
