- Dieference of PO2 between the brachial artery and dorsal foot vein in diabetics with gangrene.

The number of patients with diabetic gangrene has rcently increased in Japan. In the hope of establishing indices for predicting the onset of this disease, we performed arterial and venous blood gas analysis and plethysmography in 51 diabetics (including 12 diabetic gangrene patients) and 19 non-diabetics.<BR>The difference in Po2 between the brachial artery and dorsal foot vein was 24.1±9.8mmHg mean±S. D. in the gangrene group. It was significantly depressed as compared with 38.9±12.9mmHg in the control group (p<0.01). It was also depressed to 21.2±11.7mmHg in non-gangrenous patients with retinopathy and a duration of disease for five years or longer. It was not significantly different from the value in the gangrene group, but was significantly depressed as compared the control (p<0.01).<BR>The difference in Po2 between the dorsal foot vein and cubital vein was 19.1±8.5mmHg in the gangrene group. It was significantly elevated as compared with 1.7±12.3 mmHg in the control group (p<0.01).<BR>On the other hand, plethysmography revealed peripheral plateau waves in only 20% of gangrene patients, but disclosed no other findings specific to gangrene.<BR>There as no correlation between HbA1, and difference in Po2 between the brachial artery and dorsal foot vein.<BR>It is suggested from these results that diabetic gangrene tends to occur in patients with microangiopathy, such as retinopathy, those with a relatively prolonged duration of disease and those with elevated Po2 in the dorsal foot vein.
